CYM rules prohibit individuals in 9th grade through 12th grade from participating on a CYM parish team and any high school team (Freshman, JV, Varsity, etc.) in the same sport. PLEASE CAREFULLY REVIEW THE INFORATION BELOW BEFORE PROCEEDING. FAILURE TO DO SO MAY IMPACT YOUR ABILITY TO COMPLETE A VALID REGISTRATION AND/OR IMPACT YOUR ABILITY TO BE PLACED ON THE APPROPRIATE TEAM. Please answer the following questions before proceeding: 1. Did your child attend St. Mary Magdalen School at some point in the past? 2. Are you a registered member of St. Mary Magdalen Parish? If you can answer "YES" to any ONE of these questions please proceed with the online registration and select the SMM Alumni/Parishioner package below. If you answer "NO" to BOTH questions you must select the Non-Parishioner package below. Definition of Registered Parishioner - A registered parishioner is one who has been active, participating, and registered in St. Mary Magdalen Parish for at least one year. Note: If you receive monthly offertory envelopes through the mail this likely means you are a Registered Parishioner. Also, even if CYM has assigned your parish affiliation to be St. Mary Magdalen, this is does not mean you are necessarily a Registered Parishioner. These are two separate issues.

I hereby give my consent for the above named individual to participate in CYM athletics during the cuurent program year. I recognize that there are certain risks of physical injury in playing competitive athletics. Excluding intentional, deliberately inflicted, and illegally caused injuries, I further agree, in consideration with CYM's sponsorship of beneficial athletic competiton, to release the office of the CYM of the Diocese of Wilmington, the Catholic Diocese itself, the St. Mary Magdalen Parish, and all of their employees, directors, administrators, coaches, and staff from all legal liabilities for accidental injuries suffered by my child as a result of participation in athletic activities, or travel to and from any athletic event. Providing, however, that recourse is reserved to seek damages, medical and hospital expenses, and court costs for any such accidental injuries to my child incurred during a scheduled event from any liability insurance carrier within the limits of its liability policy. I affirm that the information above is true and correct. I agree to complete the online CYM registration, providing additional medical information as required.
